Standard Release Form
(SRF)
This form is required from all cadets. The form must be completed in it's entireity and signed by a parent or legal guardian. Additionally, no cadet can be allowed to participate in an after school activity or school sponsored trip without this form being on file. If you have any questions or concerns please contact Chief Kelly . can be allowed
Health Risk Screening
(HRS)
This form is required from all cadets. The form must be completed in it's entireity and signed by a parent or legal guardian. Additionally, no cadet can be allowed to participate in an after school activity or school sponsored trip without this form being on file. Sports Physical
This form is required from all cadets that want tp participate in an extracurricular team (drill, PT, orienteering) or qualify for a physical fitness ribbon. The sports physical clearance is good for one year, so if a cadet was cleared by a sports physical May of 2020 the cadet does not another sports physical until May of 2021. If a sports physical is on file elsewhere pls provide a copy to Eagles Landing NJROTC.an be allowed
Remote Syllabus
This form is required from all cadets. The form must be completed in it's entirety and signed by a parent or legal guardian as well as the cadet. You will be held responsible for all the information herein. If you have any questions or concerns please contact Chief Kelly. can be allowed
