Standard Release Form
(SRF)
This form is required from all cadets. The form must be completed in it's entirety and signed by a parent or legal guardian. Additionally, no cadet can be allowed to participate in an after school activity or school sponsored trip without this form being on file. If you have any questions or concerns please contact the NJROTC office for assistance. can be allowed
Health Risk Screening
(HRS)
This form is required from all cadets. The form must be completed in it's entireity and signed by a parent or legal guardian. Additionally, no cadet can be allowed to participate in an after school activity or school sponsored trip without this form being on file. If you have any questions or concerns please contact NJROTC office number.
Sports Physical
This form is required from ALL cadets especially those who participate in an extracurricular teams (drill, PT, rifle, drone and academic) and to qualify for a physical fitness ribbon. The sports physical clearance is good for one year, so if a cadet was cleared by a sports physical May of 2021 the cadet does not another sports physical until May of 2022. If a sports physical is on file elsewhere, provide a copy to Eagles Landing NJROTC.an be allowed
